Care Initiatives Hospice

Who We Are

Care Initiatives Hospice is a service of one of the largest health care networks in the state of Iowa. We are a full service, not-for-profit community- based hospice ready at any time to serve patients and their families wherever they may reside; a personal residence, a nursing facility or a retirement community. Care Initiatives Hospice is committed to providing a high quality of life to patients and their families as life's final months and processes unfold. We provide an interdisciplinary team of healthcare professionals dedicated to delivering high quality, coordinated care with compassion, respect and dignity to Iowans and their families living with serious illness and loss.

Care Initiatives Hospice began providing care in March 2009 from 6 locations in Iowa: Albia, Cedar Rapids, Des Moines, Greenfield, Sioux City, and Waterloo. Care Initiatives Hospice is a part of the Care Initiatives network of senior care providers located throughout the state.

What We Do

We strive to improve the quality of life for Iowans and their families during life's health transitions through compassionate and individualized care. Volunteers are a vital part of the hospice team and can make a big difference in someone's life. Volunteers can help by listening to memories and stories, holding a hand, reading a story, playing games, baking cookies, visiting someone at home, or helping around the office.
